Nextaff of North Country is Hiring a Clinical Services Manager-RN Near Littleton, NH

Under the direction of the Director of Clinical Services, the Clinical Services Manager will Support daily operations for the Home Health, Palliative, and Hospice programs in a safe, efficient, and effective manner. The Clinical Services Manager’s primary role will be to manage clinical quality and process and improvement within depth chart reviews and assist with telephone triage and field clinician support. The Manager will also work closely with the Director of Clinical Services and the Clinical Managers to assess patient acuity, staffing ratios, and case manager caseloads. They will work to the top of their license to problem solve, implement, and monitor processes to assist our Agency in achieving our mission and strategic goals.

Responsibilities include but are not limited to:

  • Performs clinical audits on Home Health and Hospice patient clinical records.
  • Performs clinical triage and provides support to clinicians in the field.
  • Oversees the clinical triage team to ensure they are functioning appropriately and meeting the needs of the patients and staff.
  • Reviews patient acuity at Interdisciplinary Team Meetings (IDT) to ensure all acuity is being updated and subsequent visits are scheduled appropriately.
  • Reviews staff productivity with the Scheduling Team Lead and Home Health Clinical Manager.
  • Assists with review of Agency and System policies and presents new policies to the Executive team.
  • Manages the medical records team to ensure documents and orders are processed in a timely manner and are within all compliance and regulatory guidelines.
  • Assesses visit utilization upon admission and throughout the episode for appropriateness.
  • Reviews care plans for necessity, compliance and ensures modifications are patient specific and goals/interventions are ended as appropriate.
  • Provide patient care as needed.
  • Maintains a good working relationship with community referral sources.
  • Aids in developing educational and in-service training in collaboration with the Director and program managers.
  • Establish and monitor departmental goals consistent with corporate goals and objectives.
  • Provides support to the Intake team as needed. Serves as the administrator on call when scheduled.
  • Assures compliance with all local, state, and federal laws regarding licensure and certification of organization personnel. Perform other duties as assigned.

What we expect from you:

  • RN License
  • CPR Certification
  • Copy of Negative TB Test (2 Step or IGRA Blood Test)
  • Physical (within the last two years)
  • Background Check
  • Must be fully COVID vaccinated or be exempt

Job Type: Full-time

Pay: $36.00 - $54.00 per hour

Education:

  • Bachelor's (Preferred)

License/Certification:

  • RN License (Required)
  • BLS Certification (Required)

Ability to Relocate:

  • Littleton, NH: Relocate before starting work (Required)
