Social work is a life changing journey focused on amazing patient outcomes at NewYork-Presbyterian Brooklyn Methodist Hospital. Each of our social workers push boundaries to be excellent by thriving in a compassionate culture of care. Together, we will push the boundaries of providing care to our communities and the world so we can be stronger and healthier. We are seeking a Social Worker for the Center for Community Health; Cardiology – Heart Failure and Structural Heart Programs. Join us and utilize your experience to provide a variety of psychosocial services and to coordinate continuum of care for patients and families in the outpatient setting. Assist our patients in enhancing their quality of life at home so they are able to cope with illness and daily functioning. Here, we will work together to provide well rounded care for patients beyond the hospital.

This is a full-time day shift position located at the Center for Community Health at NewYork-Presbyterian Brooklyn Methodist Hospital.

Preferred Criteria:

• One (1) year of experience in a hospital setting

Required Criteria:

• MSW degree from an accredited institution.

Current licensed Social Worker in New York State (LMSW or LCSW)
