Program Coordinator – Community Health ACN - Columbia - Days
Under the supervision of the Program Manager, the Program Coordinator works in collaboration with CCHN team members, community-based organizations (CBO), off-site practices, and community providers, to coordinate, enhance, maintain, and streamline processes of the Center for Community Health Navigation. Partner with program, clinical leaders, and community members to develop strategies and plans for patient engagement, appointment scheduling, enrollment in the hospital’s patient portal, and tracking of patient health outcomes. Coordinate, maintain, and support Center for Community Health Navigation (CCHN) projects and initiatives. Identify opportunities to improve the quality of data and program management processes with a multi-disciplinary team of experts from population health. Collaborate with program managers and project leaders to support population health, patient safety, innovation, technology, and access improvement initiatives within the Division of Community and Population Health. The candidate should be flexible in responding to changing priorities in a fast-paced environment, as well as be able to work independently and within a team environment. In addition to this, preferred candidates will have experience with and knowledge of using EHRs, and/or work experience in a hospital environment with an understanding of health disparities with adult, pediatric, and maternal health populations.

Salary Range:
