This is a non-exempt M-F 8:00-5:00pm position
The Clinical Care Gap Coordinator will coordinate care for identified individuals to address wellness protocols and plan of care update. You will be responsible for monitoring and assisting the care team in identifying and closing care gaps for clinic patients. You will have a working knowledge of payer quality programs.

High school graduate or GED with a healthcare background required
Minimum of 2 years in a healthcare related field with knowledge of medical terminology
