This is a non-exempt M-F 8:00-5:00pm position

The Clinical Care Gap Coordinator will coordinate care for identified individuals to address wellness protocols and plan of care update. You will be responsible for monitoring and assisting the care team in identifying and closing care gaps for clinic patients. You will have a working knowledge of payer quality programs.

Essential Functions:

• Coordinate care-Monitor insurance guidelines to ensure there are no gaps in patients from Newman Regional Health Medical partners
• Follow established protocols
• Review medical records and gather clinical information as needed
• Review medical records to determine care gaps
• Utilize the EMR to find documentation needed
• Collaborate with the care team
• Manage and grow all payer incentive programs
• Communicate effectively with team members, patients and outside vendors

Ideal Candidate:

• Strong organization and communication skills
• Be self-motivated and detail oriented
• Possess initiative and be able to produce standard workload

• High school graduate or GED with a healthcare background required

• Minimum of 2 years in a healthcare related field with knowledge of medical terminology
