• The Clinical Health Promotion and Education Specialist works with patients referred for assistance in treatment to prevent or reduce exposure to contracting HIV. This position provides support to providers prescribing treatment for PrEP, and monitors patient adherence to treatment. Clinical Health Promotion and Education Specialist will work with patients to understand and complete HIV risk assessments, as well as teach, counsel, and follow up with patients to manage identified risk behavior. Clinical Health Promotion and Education Specialist will also support improvements in health outcomes through coordinating care, educating patients, building trust between patients and medical providers, by enhancing communication and the continuity of care.
