Home Care Meets Flexibility: RN UAS Assessor (New York, NY)

This hybrid role offers 2 days working from home and 3 days visiting patients in the Bronx, Manhattan, and Brooklyn, Queens areas.

Make a Difference:

• Conduct in-home UAS assessments to evaluate patients' needs.
• Collaborate with the care team to create personalized plans.
• Provide education and support to patients in their homes.

Qualifications:

• NY State RN License (min. 2 years experience).
• 2 years in home health, home care, or long-term care.
• Strong assessment and communication skills.
• Compassionate and patient-centered focus.
• Computer documentation proficiency.
• Reliable transportation (on-site visits).
