New York City Health + Hospitals/Kings County Hospital Center is Hiring a Registered Dietitian Level IV Near Brooklyn, NY

Shift 9:00am-5:00pm

PURPOSE OF POSITION:

Under the joint supervision of the Senior Associate Director and Outpatient Clinical Nutrition Manager, the dietitian is to provide optimal nutritional care to adult and geriatric (ages 19 years) patients receiving services in Ambulatory Care, including the Diabetes Resource Center, B9 Practice and Diabetes Wellness Program.

AREAS OF RESPONSIBILITIES:

SUMMARY OF DUTIES AND RESPONSIBILITIES:

1. Perform full nutrition assessments including, but not limited to the following:

  • Gather nutrition history, medical history, laboratory data, medications, and meal patterns, and other relevant data.
  • Assess patient for nutrition issues.
  • Define an individualized treatment plan and develop related patient goals, and
  • Monitor and evaluate the progress each patient is making toward established goals and expected outcomes.

2. Accurately and comprehensively document results of nutritional assessments and client counseling in the medical record in accordance with department policy, Joint Commission, and CMS guidelines.

3. Provide individualized counseling and nutrition education to patients and caregivers as appropriate, including management of food/nutrient-drug interactions and therapeutic diets

4. Provide group nutrition education classes for patients using approved materials and/or curriculum, both in- person and/or virtually

5. Act as Quality Coordinator for the Diabetes Self-management Education program

6. Support interdisciplinary performance improvement goals of assigned clinics

7. Participate in unit and outpatient nutrition team meetings as appropriate and as determined by supervisors

8. Participate in outreach activities and maintain liaison with community organizations

9. Complete necessary paperwork to support insurance billing/reimbursement process

10. Complete and maintain records, including weekly/monthly productivity reports as assigned

11. Develop population-specific nutrition education materials as needed

12. Actively pursue continuing education and career development programs for professional growth

13. Be aware of National Patient Safety Goals and consistently enforce Hospital policies

14. Attend mandatory in-service training as required by Human Resources Department

15. Maintain positive and professional demeanor with patients, visitors, co-workers, and hospital administration.

16. Perform any additional duties as assigned by the supervisors defined above

Job Type: Full-time

Pay: $83,409.00 per year
