The KY MOMS MATR Case Manager advocates, monitors, and assists in planning with adults with substance use disorders to enhance the quality of life in areas of family, socialization, work/education, leisure activity and healthcare. This position will provide services and information to clients and their families regarding housing, community resources and benefits. This position will coordinate care among multiple service providers such as Drug Courts, Mental Health Substance Abuse Treatment Teams, Department of Community Based Services, and health care providers as needed. The KY MOMS MATR Case Manager is a positive team play that promotes a culture of inclusion and participation.

Required Education and Experience

  1. Bachelor’s degree in social work, psychology or, human service field
  2. Must complete a case management or service coordinator certification program offered and approved by the state within six months of employment
  3. Valid driver’s license, reliable operating transportation, and proof of current auto insurance

Preferred Education and Experience

  1. Master’s degree in Social Work
  2. Knowledge of substance use disorders, community supports and ability to work well in a team environment
  3. Knowledge of recovery-based intervention in case management services

Essential Functions

Case Management 

  1. Plan and implement general case managements functions such as: client referrals, connecting community resources, assistance with accessing public benefits, referrals for treatment and medical needs. Also, assist and advocate appropriate activities
  2. Available to meet with mothers individually to find resources and support that help to reduce stress and risk factors 
  3. Assist in a multi-faceted approach such as Community Outreach and Education, Substance Abuse Prevention, and Case Management services
  1. Provide advocacy services on behalf of clients and their families
  2. Assist clients in meeting their education, childcare and employment goals
  3. Provide psychoeducation relating to health management, recovery, natural supports and individual strengths
  4. Provide ongoing client health education, communication skills, with healthcare personnel and medication monitoring

Collaboration and Communication.

  1. Participate in state initiatives as directed by Department for Behavioral Health and complete all necessary requirements
  2. Represent company throughout various boards, meetings, and counties
  3. Inform supervisors, and other members of the team as indicated, of issues/concerns related to individual clients
  4. Participate in team and treatment planning meetings to assist clients with identifying and attaining their goals
  5. Collaborate, coordinate, and provide resources for individual clients as an extension of the treatment team
  6. Serve as a community liaison to link client to services in the community and assist clients in building family, friends, and community relationships
  7. Develop and coordinate a service plan involving all human entities involved with the clients and family.
  8. Serve as liaison between clients and the recovery team
  9. Inform supervisor and other members of the team as indicated, of issues/concerns related to individual clients
  10. This position will report and be supervised within two departments both Prevention and Treatment
