Position SummaryAccountable for assisting consumers in the Involuntary Outpatient Commitment program including: ensuring the coordination of patient care processes and outcomes. Works collaboratively with the Emergency Services staff, 2 Central, psychiatry, court system, and the State system of care. Supports the mission, values and philosophy of HMC.Primary Position Responsibilities1.Accountable for the coordination of patient care for those consumers on the case load though out their involvement in the Involuntary Outpatient Commitment Program.2.Reviews records and maintains standards of HMC, JCAHO, DOH, NJ Division of Mental Health & Addiction Services.3.Provides high quality Case Management and linkage to care.4.Constructs treatment plans for the court and monitors consumer's compliance.5.Constructs Wellness and Recovery Action Plans and Psychiatric Advanced Directives with consumers.QualificationsMinimum Education:Required:Bachelor's Degree in Social Work, Psychology, or EquivalentPreferred:Master's Degree in Social Work, Psychology, or Equivalent Minimum Years of Experience (Amount, Type and Variation):Required:One year experience in the mental health field.Preferred:None License, Registry or Certification:Required:Valid Driver's license Knowledge, Skills and/or Abilities:Required: Excellent interpersonal communication skills. Ability to identify and resolve problems. Computer literate, skilled in customer service.
