NHH is seeking a Substance Use Disorder (SUD) Therapist who will be responsible for providing group and/or individual therapy to patients to address mental health and/or substance use disorder. Other important activities include performing activities related to patient services and agency procedures (including assessments, treatment planning, discharge planning, referrals, etc.) documenting patient services, interacting with other professionals in the patient’s interest and providing necessary crisis interventions. The ideal candidate will possess knowledge and skills of treatment of population served, behavior modification, treatment planning, record documentation, de-escalation of violent client situations, individual, group and family counseling, and knowledge of human behavior.

New Horizons Healthcare is a federally qualified health center (FQHC) in Roanoke, Virginia that provides comprehensive primary care, behavioral health, dental care, and pharmacy services for almost 10,000 patients annually. Duties/Responsibilities:
Communication
  • Engage patients in the counseling process, as indicated by patient return rate, patient progress, and patient satisfaction surveys.
  • Work with patients to develop treatment plans, with specific goals and action steps designed to improve behavioral health outcomes and successfully meet the goals of treatment.
  • Communicate effectively with staff, patients, family members and community partners.
  • Articulate thoughts and ideas, including suggestions for change, in a non-threatening manner.
  • Develop excellent working relationships with NHH staff and community partners.
  • Work with supervisor and other staff in a positive and professional manner. Provide constructive feedback and communicate accurate information.
Teamwork
  • Work closely with NHH providers and behavioral health staff to develop behavioral health treatment plans that promote a holistic approach to health and provide continuity of care between departments.
  • Work with the Behavioral Health Case Manager to provide information and referral services to a variety of community services including:
    • Department of Social Services (Food Stamps, TANF, APS, CPS)
    • Emergency shelter for men and for women
    • Shelters for battered women
    • Emergency food suppliers
    • State and Federal sponsored health insurance (Medicare and Medicaid)
  • Consult with medical providers and/or psychiatrists regarding patient progress and compliance with psychotropic medications.
  • Work as member of NHH behavioral health team, attending weekly department meetings, and other functions as requested.
  • Meet with team for huddles to participate in client staffing and administrative updates.
  • Meet with supervisor as requested to discuss any clinical or administrative concerns.
Problem solving
Ability to manage time and resources; ability to prioritize patient needs and utilize resources to effect optimal patient outcomes and remove barriers; to provide patients, families, and community members quality information and guidance regarding quality healthcare and outcome metrics.
Self-Management

  • Must demonstrate a personal and professional commitment to New Horizons Healthcare and its mission.
  • Assure that any job functions that are added or changed as organizational needs require by Management are performed accurately, completely and efficiently.
  • Keep professional licenses required for the position current and up to date.
  • Maintains productivity standard of 26 sessions per 40-hour clinical work week or equivalent.
  • Keep behavioral health patient documentation up to date and current with Medicaid/Medicare standards.
Planning and organizing
  • Attends all behavioral health staff meetings, lunch and learns, and meetings as assigned by the Behavioral Health Director or other members of the Leadership Team.
  • Participates in at least 3 Community Events per calendar year in promotion of the Behavioral Health Department and NHH.
  • Participates in at least 1 internal NHH committee of personal/professional interest in consultation with the Behavioral Health Director.
  • Attends meetings or acts as back up to the Behavioral Health Director for clinical issues in his/her absence.
Technology
Utilize various technologies to provide patient care and improve outcomes; utilization of data to support and measure quality metrics; report development; computer literate with working knowledge of Microsoft Office, EMR, and various other computer software programs:
  • Utilize computer software including Excel, Microsoft Word, and eClinicalWorks.
  • Utilize ECW to make appointments, to document telephone encounters, therapy sessions and any case management activities.
  • Use articulate and professional language in all ECW documentation and any professional correspondence.
  • Completes ECW progress notes, treatment plans, quarterly reviews in a timely manner (usually same day, and no more than 24 hours from session)
  • Keep patient records up to date with required documentation for Medicaid and other insurance providers.
Learning
Expand clinical knowledge, stay up to date with current theories and evidence-based practices and meet licensing requirements by obtaining required continuing education credits.
Clinical
A. Patient Relations
  • Obtains complete behavioral health history from patient, and/or family, and/or previous medical record, etc.
  • Explains treatment to patients and families.
  • Teaches and counsels patients and families in a culturally sensitive manner.
B. Clinical Duties
  • Engage patients in the counseling process, as indicated by patient return rate, patient progress, and patient satisfaction surveys.
  • Expand clinical knowledge, stay up to date with current theories and evidence-based practices and meet licensing requirements by obtaining required continuing education credits.
  • Formulate a comprehensive mental health diagnosis, utilizing the most up to date DSM/ICD system.
  • Complete a standard mental status exam, review of symptoms and safety assessment with each session.
  • Work with patients to develop treatment plans, with specific goals and action steps designed to improve behavioral health outcomes and successfully meet the goals of treatment.
  • Educate patients regarding symptoms and strategies for behavior management related to their specific diagnoses.
  • Provide counseling and/or psychotherapy using a variety of interventions and modalities, including cognitive-behavioral therapy, short term solution focused therapy, insight-oriented therapy, symptom management, dialectical-behavioral therapy, motivational counseling, and other types of interventions as licensed and trained.
Education and Experience:
Minimum: Master’s degree in counseling or social work with appropriate licensure and at least two years of experience in behavioral health field. Maintains ethical practice as per the ACA (American Counseling Association) or NASW (National Association of Social Work) Code of Ethics.
Physical Requirements:
Work is partially performed (80%) in a normal, pleasant office environment. A portion of time (20%) will be spent in the community performing outreach and enrollment activities. Moderate physical activity is required, lifting up to 25 lbs., standing or walking for more than four (4) hours per day.
