Qualifications: Prior experience in supporting individuals with developmental disabilities is preferred. New York State drivers license is mandatory and must be insurable under New Hope Community Group Fleet Policy (see Human Resources Department for details).

Reports to: The Director of Health Services or designee

Essential Functions:

1. Take direction from the Assistant Director of Health Services or designee.

2. Obtain and maintain Medication Administration Certification, Insulin Certification and G-Tube Certification.

3. Review paperwork from all medical appointments in the homes as assigned. Ensure follow up appointments were made and noted in the appointment calendar.

4. Review and verify narcotic counts as assigned; ensure Narcotic Administration Records and Narcotic Accountability Key Pass forms are being completed as per policy.

5. Review medications cabinets to ensure medication labels match the eMAR; and that there are no expired or medication without current physician order in medication administration area. Complete weekly med orders in assigned homes.

6. Ensure supplies are replenished and available for medication, insulin or g-tube administrations.

7. Review blisters and eMAR’s for errors on a weekly basis.

8. Complete medical chart audits and annual medical reviews as assigned

9. Review monthly physicians’ orders and eMAR’s to ensure accuracy.

10. Update or create Client Specific Sheets as assigned.

11. In conjunction with residential, review adaptive equipment and check sheets to identify any concerns or needs of replacements.

12. Under Train the Trainer policy, once trained by a RN to provide training to residential on a task, can continue supporting and training on that task as assigned. (IE: mechanical lift, Epipen use, etc.)

• Occasional need to lift up to 50 lbs. independently and up to 100 lbs. with
• Occasional need for physical intervention (SCIP).
• Potential for exposure to blood and body fluids.

• Occasional sitting and computer work.
