We are recruiting for a dynamic Care Coordinator who can provide consistent quality resident care and service; delivered throughout all shifts. The incumbent is responsible for scheduling organizes and managing all aspects of a patient’s healthcare maintenance and treatment, including accepting management responsibilities of the Nursing Department in absence of the director of resident services.

Must demonstrate and promote a culture that embraces compassion, character, and commitment to every individual in the workplace. Dedicated to providing the best heartfelt care and services to our residents and their families.

Requirements

  • High school diploma or general education degree (GED)
  • Valid Nursing License or Med Tech Certification.
  • Must exhibit superb communication skills.
  • Takes initiative in work, seeks out additional information and offers solutions/suggestions.
  • Shares on-call responsibilities/shifts as required
