The State of New Hampshire, Department of Health and Human Services, Division of Public Health Services has a full time vacancy for Heart Disease Program Coordinator.

SUMMARY:

Manages administrative and program operations of heart disease programs to reduce the burden in the State of New Hampshire. This position serves in the Department of Health and Human Services, Bureau of Prevention and Wellness, Chronic Disease Prevention and Screening Section.

RESPONSIBILITIES:

Plans, develops and evaluates program goals, objectives, and activities to ensure coordination of services and implementation of science-based public health practice to reduce the burden of cardiometabolic conditions.

Coordinates with, and convenes healthcare, insurance, health information technology, professional organizations, and community stakeholders to develop and recommend policies, procedures, standards, and guidelines to ensure optimal disease management, to reduce the burden in the New Hampshire population.

Serves as a subject matter expert in public health interventions to improve cardiometabolic health, able to critically review surveillance data, peer reviewed literature, position statements, evidence-based guidelines and apply to practice.

Coordinates program operations including managing contractors, consultants and work of other professional staff to achieve Department objectives related to improving cardiometabolic health of the NH population.

Plans, implements, presents, and evaluates continuing education programs, in-service trainings; provides technical assistance develops materials, reports designed for healthcare and public health professionals on topics related to cardiometabolic health.

Assists with the development of performance measures, and the collection of data relevant to the program evaluation. Uses data and evaluation results to improve program performance and training materials.

Monitors grants and cooperative agreements to ensure compliance with government regulations and assists with the preparation of federal grant applications and reports to assure continuation of federal program funding.

MINIMUM QUALIFICATIONS:

Education: Master's degree from an accredited college or university program with a major study in health sciences, such as nursing, nutrition, exercise physiology, social work, or related field. Each additional year of approved formal education may be substituted for one year of required work experience.

Experience: Four years’ experience in a public health or healthcare setting with responsibility for program planning, monitoring and evaluation. Each additional year of approved work experience may be substituted for one year of required formal education.

License/Certification: Valid driver’s license for in state travel.

PREFERRED QUALIFICATIONS: The successful candidate must have experience facilitating coalitions, learning collaboratives or other multidisciplinary stakeholder groups. Some knowledge of chronic diseases and risk factors is helpful.
