Direct Support Staff/Personal Assistant

Position Summary: Position providing direct home and community based support to a 49-year old man who has cerebral palsy, cognitive disability, and is a wheelchair-user; his receptive language is significantly stronger than his expressive language. Responsibilities include 1:1 direct support with daily care tasks, activities at home and in the community, transportation, assistance staying safe, engaging in tasks and activities beyond just companionship, and support with making progress towards his goals. Several shifts are available on all days of the week and hours are flexible.

Location: San Jose, CA

Compensation: $50-$60 per hour

*Mileage reimbursement for any client transportation (using family’s adaptive van)

Work Schedule: Very flexible. M-F afternoons

Weekend shifts a plus, but not mandatory

*Approximately 30 hours a week needed - this can be either designated to one person or split between two providers if you are seeking fewer hours/part-time work.

Qualifications and Characteristics:

• Trained in safety care
• Experience working with individuals with limited expressive language skills
• Observant of details, responsive, reliable, and able to initiate new tasks and activities
• Flexible, adaptive, and open to learning more about the client and how best to work with him, including support needs and communication system
• Patience - go at the client’s pace and respect the need for transition time
• Strong communication skills
• Consistent and committed - punctuality is a must
• Willing to regularly implement stretching/strengthening routine
• On the lookout for community events/opportunities he can engage in (ex. sports, music, mariachi, car shows, and horse racing events)
• Little to no cell phone use during work hours
• Perineal care
• Experience with g-tube usage (administration of medication and formula) or openness to training
• More active involvement than just companionship - check in on what he needs/wants. Example: He cannot just go up to a concession stand to buy what he wants while out in the community, so please ask.
• Clean driving record/driver’s license
• Willingness to engage in musical activities - using musical instruments, games, etc.
• Able to swim and support the client safely in the water is a plus, but not required
• Need to be open to tracking hours and mileage through a financial management service (funding source for California’s Self-Determination Program)

Requirements and Core Responsibilities:

• 1:1 direct support with activities, including prompting with visual aids and transition objects. Client needs full assistance with some tasks, minimal support with others.
• Assistance with activities of daily living, including hygiene (shaving, cutting nails, bathing using a shower chair), administering formula via g-tube, light housework, and meal prep.
• Transportation to and from preferred activities using the family’s adaptive van.
• Assistance staying safe out in the community.
• Consistent communication with parent and other support staff.
• Changing diapers and maintaining hygiene/perineal care
