Summary

The RN Case Manager is a member of the interdisciplinary team who provides timely, quality and cost-effective coordination of rehabilitation services to best meet the needs of the individual served. The Case Manager promotes positive outcomes, serving as a link between patient, team, payer and community and other stakeholders as appropriate. The case manager facilitates a plan of care that promotes patient-family decision making and maximizes service delivery transitions for post-acute rehabilitation care. 

Provides clinical, casework, and group work services to clients and their families by performing the following duties.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

  • Handle client case assignments
  • Communicate between doctors, nurses, interdisciplinary team, clients, families and other stakeholders as appropriate.
  • Work with pharmacies regarding denied medications for our clients
  • Coordinate and request DME for our clients
  • Relays pertinent medical information, treatment plan, goals, progress and discharge planning needs in a proactive and timely manner with payers, attorneys and other vested stakeholders.
  • Coordinate medical appointments for clients in the community and filling out all pertinent paperwork for scheduled appointment
  • Coordinate, lead and take minutes for PCC on all Client Goal Plans, Team Conferences and Discharge Conferences
  • Review medical records for new admissions and completes plan of care assessment within 48 hours of admission
  • Balances the role of patient-family advocate with the role of care coordination. Promotes patient-family decision-making to the greatest extent possible in goal-setting, discharge planning and resource allocation. Fosters positive and professional relationships across all parties including but not limited to patients, families, attorneys, physicians, staff, peers and external stakeholders. Interact with clients and families personally at least one time per week to assess goals and progress and to ensure satisfaction.
  • Be the initial and primary point person at NeuLife for clients for any needs and concerns, intervening in crises, and provide personal support
  • Coordinate out of town visitors for our clients
  • Attend daily stand up meeting
  • Round with Physician
  • Communicate client’s progress and notify team of any concerns and needs by reporting at daily interdisciplinary meetings, Team Conferences and Grand Rounds 
  • Prepare client discharge; coordinating discharge and post-discharge requirements as per NeuLife guidelines
  • Prepare and maintain reports that track clients and their conferences
  • Back-up to Field Clinical Liaisons for Nursing Assessments for possible admissions
  • Gather reports from all disciplines and draft Monthly Client Reports
  • Take calls as they come in (family members, attorneys, outside doc, medical director, staff with client issues)
  • Greet client at admission process and conduct orientation

Education and/or Experience

Associate's degree or equivalent from an accredited college or university and two to four-year experience as a case manager. Experience with workers’ compensation and commercial plans preferred. Bi-lingual preferred.

Certificates, Licenses, Registrations

Valid and unencumbered license as a Registered Nurse in the State of Florida. #ZR
