Nemours Children's Health is Hiring a Social Worker - Licensed Clinical Therapist Near Wilmington, DE

Job Description
Nemours is seeking a Social Worker to join our Nemours Children's Health team in Wilmington, DE.
This position is part of the social work department and works to support children with acute behavioral health/ psychiatric needs when they present to Nemours. Most of the work is done in conjunction with the Psychiatry, Emergency Department and Inpatient medical teams. The focus is on risk assessment, safety planning and consideration of other levels of care. This social worker would assist with planning for a safe discharge with an appropriate follow up plan and would work closely with internal and community stakeholders as well as the child and family.
Essential Functions
  1. Attend patient care unit and/or Medical division rounds in accordance to team expectations and provide inpatient admission screening on assigned patient care unit.
  2. Manage complex assessments of the pt/family's biopsychosocial situation establishes goals and periodically re-assesses the pt/family situation.
  3. Provide crisis intervention services where indicated for the management of domestic violence, psychiatric emergency cases, child abuse and family disputes.
  4. Provide therapeutic, goal directed counseling. Exhibit necessary clinical skills to provide problem focused goal directed intervention.
  5. Interface with insurance providers, case managers and others involved in the authorization of needed patient services. Identifies and negotiates differing needs within the family system
  6. Coordinate discharge planning needs of patients and their families in an efficient, effective and timely manner while including the family in the process.
  7. Establish and maintain effective working relationships across disciplines and coordinate family meetings. Coordinate recommendations of team meetings.
  8. Document social work notes in the patient medical record within the guidelines of the Department policy.
  9. community resources and provide resource specific information to the assessed needs
  10. Maintain a professional manner at all times with staff and family.
Qualifications
Masters Degree in Social Work from a program accredited by the Council on Social Work Education (CSWE)
6 months job related experience
Graduate school internship may be applied to the job-related experience at the discretion of the Senior Director.
