What you will do:

This position acts as the first point of contact with all referral sources, community agencies, medical providers, potential participants and families, and provides intake, information and referral as required. Acts as office manager, when appropriate, and is responsible for ensuring adequate office supplies are ordered or coordinated with Main Office staff, conducts file maintenance, faxing, mailing, scheduling and telephones. Provides support services to the CM/HBCW Case Managers and RNs as assigned. Under the direction of the Clinical Quality Supervisor, gathers data and assists in preparing reports for a variety of quality assurance functions.

  • Provide initial intake, information and referral assistance to referral agencies and program participants;
  • Input data for any program operated by CM/HBCW for office tracking and quality assurance purposes;
  • Maintain participant files;
  • Responsible for all office supply ordering or coordinating orders with Main Office staff;
  • Assist with scheduling assessment and reassessment appointments with SC input, as needed;
  • Responsible for the follow-up and monitoring of service status including monthly MI Choice Program Wait List calls as needed;
  • Maintain a professional and positive working relationship with referral sources, providers and program participants;
  • Maintain monthly Closed Tracking Log and assures data is complete, accurate and submitted to AAA Director in a timely manner;
  • Ensure adequate supply of program forms and new participant packets are assembled and available;
  • Coordinate all paperwork prior to submission to Main Office and providers;
  • Responsible for documenting communication in participants’ files in accordance with state and federal documentation guidelines;
  • Submit required reports within appropriate timeframes;
  • Adhere to personal Continuing Education Plan (CEP);
  • Provide back-up assistance to satellite offices as needed.
