Job Summary

Community Health Long Term Care staff support Neighborhood House’s mission framework and Strategic Plan by contributing to helping older adults, individuals with disabilities, and their caregivers receive the services and supports needed to remain in the community. The primary responsibility of the Training and Education Coordinator is to provide on-going coaching for staff that supports effective case management support and home visiting practices that lead to positive outcomes for clients within the context of a collaborative relationship with caregivers, case managers, and families. This position will report to the Community Health Program Director.

Responsibilities

Program Planning, Design, and Implementation (40%)

  • Anticipate future trends and articulate credible visions enabling innovative and appropriate program planning. Design, implement and evaluate program development strategies.
  • Support the supervisors and managers in determining whether the program is able to achieve compliance and quality benchmarks.
  • Complete all required program reports accurately and in a timely manner.
  • Manage access requests for all program staff to relevant program systems.

Promote and Support Case Management and Client Needs (60%)

  • Develop and maintain positive, supportive relationships with program staff for the purpose of supporting staff growth through the mentoring process.
  • Develop and maintain schedules for visits and observations for the purpose of determining needs.
  • Utilize Relationship Based Coaching cycle components for the purpose of assessing needs, engaging in focused observations, and providing reflection and feedback in order to set goals and develop action plans.
  • Provide constructive feedback and support for case management and home visiting practices for the purpose of increasing case manager skills and competence. This may include modeling, co-home visiting, observing, providing reading materials or other resources as needed.
  • Support case manager implementation of program’s approach to culturally responsive programming for the purpose of meeting the key needs of all clients.
  • Gather and record information about case manager’s implementation of practices described in goals/action plans for the purpose of summarizing and analyzing data.
  • Plan and present individual and group workshops on relevant topics based on program data for the purpose of providing individualized professional development.
  • Provide follow up support to staff following any form of training for the purpose of increasing implementation of information and transfer of skill into practice.
  • Work jointly with Managers to compile and interpret program data for the purpose of determining focus areas for mentoring and professional development as well as developing data related procedures as needed.
  • Participate in substantial initial and ongoing professional development
  • Provide effective mentor/coach activities as well as provide expertise on theory and practices that support case management work.Perform other duties as .

Qualifications

Minimum Qualifications

  • Three years’ experience in case management with older adults or individuals with disabilities.
  • Three years supervisory experience in a related setting, including group organizing and working with families.
  • Knowledge of and familiarity with the practices and principles oflong term care case management, including assessment, support services, promoting caregiver relationships, continuity of care, and case manager training and supervision.
  • Knowledge of principles and methods for curriculum and training design, teaching and instruction for individuals and groups, and the measurement of training effects.
  • Experience with data collection, analysis, and reporting to stakeholders including staff, program management and program leadership.
  • Good interpersonal and communication skills, including the ability to work as part of a team, communicate effectively both orally and in writing, receive and provide feedback, and manage dynamic interchanges in meetings.
  • Willingness to comply with established agency performance standards which may include: productivity/personal responsibility, client/customer service orientation, embracing a team player orientation, entrepreneurialism, and maintaining a professional demeanor.

  • Establish a professional development plan and participate in ongoing professional development.
  • Must have reliable transportation and be able to travel independently to Neighborhood House service sites and client homes throughout King County.
  • This position will require 2-3 days onsite per week at the program’s Tukwila office.

Desired Qualifications

  • BA/BS in social work or related field; Masters Degree preferred.
  • Familiarity with and experience implementing Title XIX Long Term Care Case Management and Medicaid Transformation Project MAC/TSOA performance standards preferred.
  • Bi-lingual ability.

Required Credentials

  • Must pass Neighborhood House background check requirements.
  • Position requires Washington State bi-annual criminal background re-check.

Post-Hire on-the-job Training Requirements

  • Child Abuse and Neglect training (within 2 weeks of hire).
  • Current First Aid Certificate within 30 days of hire.
  • Current CPR Certificate within 30 days of hire.
  • Sexual Harassment Training within 30 days of hire.
