PRIMARY FUNCTIONS:
The Breast Health BilingualPatient Navigatorwill work with Neighborhood Healthas a lead staff member of the FOCUS Breast Cancer Screening and Linkage to Care Program, a pilot program aimed at advancingtimely diagnosis of breast cancer in vulnerable populations. They will work with NH patients who have increased risk of breast cancer due to family history, ethnic background, breast symptoms, or abnormal breast cancer screening to ensure they receive complete evaluations and risk reduction counseling. They will complete and document breast cancer risk assessments on potentially high-risk patients. They willassist patients in completing full diagnostic evaluations including all necessary imaging studies, biopsies, and genetic testing by providing care coordination and patient education. They act as a patient advocate to ensure proper coordination of care and closure of healthcare delivery loops.They demonstrateexcellent empathy, cross-cultural communication skills, and cultural competency.They are responsible fornavigation tracking and data management and reporting.
Incumbent must have a deep interest in women’s health, bilingual in Spanish and English preferred and have a strong commitment to Neighborhood Health’s mission to improve health and advance health equity by providing high-quality primary care regardless of ability to pay.
DUTIES/ RESPONSIBILITIES: (Essential Functions: Duties and responsibilities noted with bullets are the essential functions of the job).
Assist breast health patients with navigating the health care system to ensure access to care. This includes but is not limited to diagnostic mammograms, breast ultrasounds, breast MRI, breast biopsies, genetic testing, and consultations with specialty providers.Reach out to patients via phone and text to help them schedule mammogram and other appointments at radiology offices.
Fax mammogram, breast sonogram, breast, and MRI biopsy orders or reports,as needed.
Send text reminders to patients with appointment information, including location and instructions, as needed.
Track completion of all diagnostic testing and ensure results are received and reviewed by ordering provider.
Discuss patient findings (after provider review) with patient and explain next steps in patient care journey.
Assess and address patient barriers to accessing breast health care
Perform escalated case management duties for vulnerable, or high-risk breast care patients to include assistance with financial aid applications, specialty health care appointments and prescription drug assistance.
Follow up with difficult-to-reach patients, assess reasons for declining care, identify patterns and barriers, and reschedule no-shows, as possible.
Assist patients in need of interpretation services and transportation services.
Communicate with all members of the patient’s Neighborhood Health care team on behalf of the patient.
Coordinatepatient care within Neighborhood Health system and by linking patients to care with Neighborhood Health external partners (Inova/UVA/Virginia Hospital Center).
Implement breast cancer risk assessments to identify patients at high risk for cancer; including directly assisting patients with risk assessment completion and documenting assessment results in medical record.
Serve as a breast care information resource to members of patient care team.
Participate in regular meetings with quality manager and chief medical officerto review cases,data and discuss plans to reach targets and to troubleshoot challenging cases.
Collect and document project data using EMR and population health tools, and report to funder monthly.
Train and Oversee volunteers to assist with breast health program
QUALIFICATION/EDUCATION and/or EXPERIENCE REQUIREMENTS:Required Experience
Bilingual skills highly preferred(Spanish/English)
BSN Nursing degree orbachelor’s degree in public health, social work, or similar field
Experience as a Breast Health Patient Navigator preferred
Knowledge of breast cancer diagnostic evaluation.
Strong organizational skills; attention to detail and accuracy
Good interpersonal, communication, and customer service skills
Compassionate and Caring demeanor
Good typing skills and proficiency in Microsoft Office programs, particularly Excel
Experience working with Electronic Medical Recordsystem (training provided)
Previous experience working with vulnerable communities, particularly the uninsured/Medicaid population
Ability to work under minimal supervision with multiple internal and external partners collaboratively and supportively.
