Near North Health, a longstanding pillar of Chicago's healthcare landscape, is seeking a visionary and compassionate Licensed Clinical Professional Counselor (LCPC) to join our dedicated team. As Licensed Clinical Professional Counselor (LCPC) you will play a pivotal role in our mission to serve primarily low-income uninsured or underinsured residents of Chicago's medically underserved communities. Your systematized routine for responding to referrals for patients receiving behavioral health services, will further solidify our position as one of the largest providers of community-based primary care in the city.


Key Responsibilities
:

  • Facilitate Care: Provide individual patients with the psychosocial support needed to cope with psychosocial stressors and/or medical issues.

  • Support Medical Providers:
    Assist medical providers by providing supportive therapeutic and crisis-response interventions as required.

  • Formulate Assessment:
    Conduct mental health assessments for referred NNH patients, including EIS, PGM, and other integrated behavioral health clinics as needed.

  • Meet Health Care Goals:
    Ensure and meet all Domestic Violence program goals, objectives and activities in accordance with NNH Mission and Health Care Plan and applicable funding requirements. (after certification received)

  • Implement and Coordinate:
    Establish, maintain and conduct a routine system for screening patients who present with mental health and/or psychosocial issues.
  • Master’s Degree in Counseling from a Counseling by a program accredited by the Council for Accreditation for Counseling and Related Educational Programs.
  • Current State of Illinois Licensure
  • Written and verbal fluency in the required languages.
  • 3 years of experience in the field of social work (internships qualify). Must have the ability to: take direction/supervision; willingness to ask for clarity in ambiguous directives or situations; ability to establish and maintain professional working professional working relationships with staff and other professionals; ability to provide leadership in the areas of clinical relationships with staff and other professionals; ability to conceptualize and transmit knowledge and techniques in the resolution of problems.
