The Care Coordinator conducts comprehensive substance use and mental health disorder assessments, makes treatment referrals, and provides intensive clinical case management services.

Master's Degree in counseling, social work or related field. LPC, LCADC or LCSW with co-occurring experience strongly preferred or documented evidence that an associate license or full license will be obtained in the next year. License/Certification: CADC, LCADC, LCSW or LPC (Preferred).

Must have a minimum of two years co-occurring disorder experience and applied knowledge of The ASAM Criteria, 3rd Ed., and the DSM-5-TR.
