POSITION SUMMARY

This position supports our palliative care program titled Integrated Care. Responsibilities include providing psychosocial support, care coordination, and community resource options to participants and their families. The Integrated Care Social Worker is also responsible for routine follow up through a non-medical care coordination and risk stratification care pathway for participants through requests of community referrals and coordination of provider visits.

Primary Responsibilities:

  • Provide critical psycho-social support to participants through telephonic assessments
  • Work closely with the palliative care team in the clarification of goals of care and plan of care (attending physician, staff, patient, and family)
  • Establish strong organizational system for tracking participants with timely/appropriate follow through
  • Serve as an 'expert' in community resources to ensure proper care coordination and comprehensive patient transition in an outpatient setting
  • Provides crisis intervention, individual, couple and group therapy to patients and families for a diverse range of issues related to grief/loss, adjustment to illness, relationships/family dynamics, mental health, abuse, symptom/pain management, and spirituality
  • Participates in the provision of social work services in a transdisciplinary care team setting; contributes/participates regularly in IDG team conferences and collaborates with team members in developing a patient centered plan of care.

Knowledge and Skills Requirements:

  • Familiar with concepts and practices used in a palliative care setting
  • Expertise in written and oral communication
  • Demonstrated ability in documentation requirements, service coordination and case management
  • Demonstrated leadership in health care related organizations
  • Ability to develop and implement long and short-range organizational goals
  • Ability to work harmoniously with a broad range of staff, volunteers, and the community as a representative of Hospice Hawaii

Experience and Educational Requirements:

  • Current Hawaii LCSW License and minimum three (3) years direct patient care experience required
  • At least two (2) years of experience in healthcare, internal medicine, geriatrics, oncology, or hospice/palliative care
  • Advanced certification in Hospice & Palliative Care preferred

Equipment Used:

  • Frequent use of own personal motor vehicle which meets valid registration, safety and insurance requirements. Must possess a valid Drivers' License, current auto insurance, and reliable transportation

Hours:

  • Generally Monday through Friday, 8:00 am through 4:30 pm.
  • May include after-hours work including weekends and holidays, as needed

Physical, Mental and Communication Demands:

  • Requires long periods of sitting/sitting while driving
  • Requires use of hands and must be able to speak, read and write English. Vision abilities include close vision, depth perception, and ability to focus.
  • Must be able to lift and move up to 25 lbs unassisted
  • Must be able to work on multiple tasks simultaneously and meet multiple deadlines as well as analyze problems and adopt an effective course of action. Requires learning a number of policies and procedures, writing, arithmetic and problem0solving skills
