Position Summary:
The Screening, Referral and Data Coordinator will be responsible for the coordination of internal requests for screening sent to the housing program; brief screening and referring households to appropriate resources; coordinating with local Coordinated Access systems to accept referrals for housing services; and providing administrative support including data entry and generating reports using the HMIS database.


What you will do:

  • Maintain accurate and updated documentation of housing program services in HMIS database; ensure documentation is complete and run regular reports to check for data quality.
  • Coordinate the collection of service data with housing program staff.
  • Accept and coordinate referrals from service providers and homeless households in need of housing resources.
  • Screen for eligibility and triage referred households (families and individuals), ensuring households are referred to appropriate resources.
  • Address any immediate safety concerns experienced by referred households, in consultation with supervisor.
  • Attend Coordinated Access for Adults meetings to provide/receive information on referrals as needed
  • Demonstrate a person-centered approach in working with clients by respecting their self-determination and autonomy
  • Consult with supervisor in complex cases
  • Participate in agency and housing team meetings to coordinate activities as needed
  • Strongly adhere to Trauma Informed Care and Assertive Engagement principles
  • Experience with computers and database systems required; ability to create Excel spreadsheets and fillable PDF forms; experience with HMIS is a plus.
  • Experience working with homeless adults and/or families and demonstrated understanding of multiple barriers frequently experienced by homeless households.
  • Knowledge of local resources for individuals experiencing homelessness, including shelter, housing, food, clothing, behavioral health, and medical resources.
  • Experience working with Native American communities strongly preferred.
  • Demonstrated experience paying excellent attention to detail and accuracy
  • Strong organizational skills: ability to keep track of multiple tasks
  • Strong written and verbal communication skills with team members, NARA staff and with individuals seeking services
  • Ability to work independently and as part of a team
  • Ability to maintain professional boundaries with clients, the community, and other staff members, and to work collaboratively and always treat everyone with respect and dignity.
  • Demonstrate compassion and a commitment to equity.
  • Demonstrated ability to develop rapport and trust quickly and effectively with individuals
  • Must be consistent in the implementation of screenings and be able to respond quickly to requests for assistance, while clearly communicating expectations and limitations.
  • QMHA, Certified Recovery Mentor (CRM) and/or Peer Support Specialist certification preferred
  • NARA NW is fully committed to supporting sobriety. If in recovery, must have been sober/clean for a length of time sufficient to demonstrate that will be able to model sobriety.
  • Ability to pass a pre-employment or for cause drug tests.
  • Ability to pass DHS criminal background and DMV checks.
