We are excited to announce our new Home Base Youth Shelter Services Program opening November 2022 in Surprise, Arizona. We are currently recruiting for a full-time Case Manager. Improving the lives of individuals and families through Native American culturally appropriate behavioral health, affordable housing, and community development services.

POSITION SUMMARY:

The position of case manager provides the necessary stability to the residents of the property through initial move in orientation of community rules and program rules, case management services, and enforcement of all property rules and state and federal laws for residents residing at Native American Connections properties. The ideal candidate will be someone who enjoys working independently, generates new ideas, easily aligns with the program philosophy and goals.

RESPONSIBILITIES:

• A full understanding of NAC’s philosophy of housing and resident wellness
• Enter case management information in HMIS software program for case management data.
• Complete initial case plan and SPDAT within 3 days of move-in; complete intake forms with youth and evaluate the level of need for workforce case mgt services within 3 days of move-in; complete and update resident case management plans and files at least weekly.
• Administer the SPADT assessment tool in suggested intervals.
• Connect Youth to community resources that offer life skill and employment assistance.
• Maintain strong working relationship with NAC outpatient services and refer for behavioral health counseling as appropriate
• Provide referrals for emergency stabilization as needed; provide referrals for and/or deliver Financial Education; referrals for food boxes; manage bus pass program; refer (and follow-up) to DES, applications for food stamps, AHCCCS as needed; work individually with clients to maintain employment.
• Maintain timely, accurate case management written file documentation on Youth’s attendance, services, goal progress, rental history and recertification of income.
• Attend weekly staffing to update all residents progress
• Participate in Youth Care Coordination meetings
• Maintain daily shift report and document all resident interactions.
• Conduct monthly community meetings and work to improve the payment history, health and safety issues, and resident conduct through the community through one on one education of the residents as needed.

EDUCATIONAL REQUIREMENT:

• Bachelor’s degree preferred.

WORK EXPERIENCE/SKILLS REQUIREMENT:

• Two years experience in service delivery OR any combination of related education, professional training or work experience which demonstrates the ability to successfully perform duties
• Knowledge of the economic, educational and social problems of Native Americans and referral services
• Experience working with Transitional Age Youth (TAY) population preferred
• Supervisory role preferred
• Must be able to work well with others in a team approach
• Excellent communication skills – written and oral
• Experience in working with the Native American population preferred
• Possess and maintain a valid Arizona drivers license and reliable transportation and clean driving record
• MS Office skills
