Nationwide Children's Hospital is Hiring a Homecare Social Worker - Northwest Region Near Maumee, OH

Overview

The Homecare Social Worker - Northwest and Columbus functions as a member of both the Toledo/Northwest Ohio Nationwide Children's Hospital Homecare team (primary) and acts as a remote social work support to the Columbus Homecare teams. The social worker provides clinical social work services to patients and their families receiving services for nursing or clinical therapies. The social worker will work at a northwest Ohio Nationwide Children’s Hospital location. Periodic travel to Columbus may be requested.

Responsibilities

  • Provide appropriate and comprehensive assessment, diagnostic and therapeutic services to patients and families in homecare clinical programs.
  • Identifies issues in practice that warrant increased investigation in order to better serve the needs of the homecare patient and family and participates in relevant research activities that will provide information necessary for more effective, efficient, collaborative delivery systems.
  • Participates in community health and social work networks that facilitate the provision of coordinated, interagency services.
  • Balances cost, quality and mission in all decision making.
  • Supports agency-wide patient care and case management activities as appropriate to the role.
  • May periodically provide remote back-up social work coverage for the hospice and palliative care social worker(s) who are based in Columbus.
  • Participates in hospital, homecare, and social work staff in-services.
  • Provides direction and support to Homecare Northwest staff in dealing and coping with their feelings about illness, loss, and death to facilitate optimal team functioning.
  • Collaborates with the interdisciplinary team and the family in the development of appropriate family centered care plans following the Medicare Condition of Participation Guidelines.
  • Identifies and, when necessary, coordinates community resources that support optimal family function for patients receiving homecare services. This could include assisting families in applying for Ohio Waiver programs, HCAP applications, school advocacy, and other like options.
  • Appropriately documents interventions in accordance with JCAHO standards, Medicare Conditions of Participation, and Clinical Social Work standards.
  • Attends scheduled interdisciplinary team meetings and provides clinical input as warranted.
  • Acts as Homecare Northwest and Columbus service area liaison with various Ohio counties’ Child Protective Services Programs as warranted.
  • Pursues educational endeavors that will support continued professional growth for self and team.

Qualifications

Education: Master’s degree in social work from an accredited school of social work.

Licensure: Licensed with the State of Ohio Counselor, Social Worker, Marriage and Family Therapist Board. Valid Ohio driver license in good standing, proof of sufficient auto insurance, reliable personal vehicle required.

Certifications: N/A

Skills: Possess well-developed coping skills. Strong ability to problem-solve with families, staff and other community agencies.

Experience: Two years of previous medical social work experience preferred.
