Responsibilities Of The Nurse Practitioner - Health Risk Assessments
- Conduct comprehensive in-home or remote health risk assessments on Medicare Advantage members
- Identify all active and chronic disease conditions to determine physical, mental, and social needs during the visit
- Educate and counsel patients on identified conditions and screening procedures
- Document findings using the provided iPad
- No continuum of care, prescribing, on-call, or follow-up is required
- Active and unencumbered Nurse Practitioner license
- Minimum availability of 10 hours per week is required to qualify
- Mileage (if applicable), training, and no-show visits are paid
- Travel Radius - Flexible; more availability and more of a radius will get a priority
- Zip Code exclusion - Yes, you can exclude specific zip codes
- No contact with the patient's PCP - notes are sent electronically
- A collaborating Physician is assigned, and Malpractice insurance is provided
