Major Duties and Responsibilities

1. Review patient handout materials and provide suggestions for improvements.

2. Engage with underrepresented patients through phone calls &/or in person interviews.

3. Review patient handout materials and provide suggestions for improvements.

4. Member will meet with BI Team for outreach activities. Member will flag patients with hypertension, diabetes, HIV/AIDS for follow-up. Member will review existing treatment plans for high-risk patients aged 50-plus.

5. Member will utilize results of the Needs Assessment survey to develop
recommendations. Member will serve with consultants conducting our tri-annual Needs Assessment Survey. Member will review existing needs and compare them to the ECM model to make a recommendation as to whether One Community Health should participate in the ECM program.
