Role: Patient Coordinator
Duration: 5 Months
Location: Fort Carson, CO (100% Onsite)

Description:
Required Skills:
Detail oriented, telephone manner, ability to work independently
Preferred Skills:
"Understanding of healthcare concepts (i.e., access to care) and general referral management processes
Previous Défense Health Agency or DoD (Department of Défense)Experience preferred to expedite the badging process "
Day-to-day Responsibilities:
Call patients to support with scheduling medical appointments. Coordinate with local healthcare professionals to confirm appropriate process for patient and potentially identify capacity to see patient within the Military Health System.
Liaise with Health Net staff to trouble sheet issues with referral authorizations.
Expected Deliverables:
Several (~12 ) daily calls to patients to help coordinate their specialty care. After each interaction with patient, complete Questionnaire containing questions on patient's engagement.
Education:
Bachelor's Degree
Project Description:
Provide Patient Navigators. The centralized referral management agent centrally and efficiently reviews and dispositions specialty care referrals based on guidance from the specialty care leader. The navigator takes over the case when the patient requires any additional support due to lack of access within the direct care system leading to deferral to the network, care across multiple aspects of our system (direct care, private sector care, VA care etc.).
