Position Description: Corrections Navigation Specialist

Position Type: Full Time/Non-Exempt

Supervisor: EIS and Intake Coordinator

Location: Nashville, TN

As a Corrections Navigation Specialist at Nashville CARES, you will develop relationships with clients and support them through the process of exiting incarceration or returning to incarceration to ensure they maintain access to medical care, as well as a range of services needed to establish stability.

What You’ll Do

· Provide pre-release HIV linkage services for people living with HIV who are incarcerated in jail/prison

· Completes eligibility evaluation, including gathering eligibility paperwork and completion of Ryan White Eligibility System enrollment

· Maintain complete and accurate documentation of services provided in compliance with agency confidentiality and quality assurance standards, Ryan White program requirements and enter all required data into reporting system

· Represent Nashville CARES as the first contact with individuals seeking services from Nashville CARES

· Complete psychosocial assessment and initial plan of care that supports people living with HIV in connecting to medical care within 30 days of diagnosis or identification

· Establish and maintain relationships with current and potential correctional, community, medical and social services providers to increase continuity of care

· Coordinate and provide services that strengthen the capacity of individuals living with HIV exiting the correctional facility to achieve successful community reentry with a focus on addressing barriers to reentry and services that facilitate maintaining/achieving positive medical outcomes

· Provide education and resources at agency and program events as well as clinical screening sites as appropriate

· Create and monitor individual client care plans to remove barriers to sustained HIV medical services

· Conduct comprehensive health/risk assessments for clients who reside in and exit the correctional facility; including readiness for care, service planning, monitoring, and follow up activities based on need

· Work with internal team of care providers to identify and establish contact with key populations and with correctional, medical, and social services providers in the community to enhance system navigation for people living with HIV

Minimum Requirements:

· Bachelor’s degree (preference for Bachelor’s in Social Work or other Human Services)

· Minimum of two years’ experience in correctional settings or community based social or health care services

· Proficiency with a variety of technology, including Microsoft Office software (Word, Excel, Outlook), electronic health records (training provided), and general Windows PC and cell phone technology

· Must be able to pass prison and jail background check consisting of NCIC, TCIC, and FBI to enter facilities

· Valid Driver’s License, vehicle, and automobile liability insurance to facilitate local travel required for client/agency-related work

Pay Range: $38,000-$42,000 annually ($18.27-$20.19 hourly).
