Namaste Home Health and Hospice is Hiring a Hospice Clinical Manager - RN Near Denver, CO

JOB SUMMARY The Clinical Supervisor is responsible for ensuring that patient care is coordinated and managed appropriately. The Clinical Supervisor is responsible for ensuring that care and services are delivered appropriately and for the supervision of clinical personnel. DUTIES & RESPONSIBILITIES 1. Receives case referrals. Reviews available patient information related to case, including home visits, to determine hospice/home care needs. Assigns appropriate hospice/home health personnel to case as needed. Conferences with MEDICAL DIRECTOR regarding any questions about an individual's eligibility for services. 2. Responsible for understanding and assuring the development, implementation, and updates of the individualized plan of care, which would entail communication with all physicians involved in the plan of care and integration of orders, including medication orders, from all physicians involved in the plan of care when needed. 3. Assists field staff in establishing immediate and long-term goals in settings priorities and in developing plan of care. 4. Triage calls from facilities, patients, families, and doctor offices. 5. Address complaints and concerns from facilities, patients, families, and doctor offices. 6. Provides workflow approval for home health and hospice. 7. Reviews and evaluates each case through a variety of means such as home visits, conferences, record review, and the services provided by clinicians; discusses and verifies impressions, instructs, and guides clinicians to promote more effective performance and delivery of quality home care services; and is available at all times during operating hours to assist clinicians as appropriate. 8. Reviews patient's medical diagnosis, prognosis, medications, procedures, and clinical course. 9. Attends case conference meetings with hospice/home health personnel to facilitate coordination of care and discussion of interdisciplinary group involvement as required. 10. Conducts quarterly record reviews and communicates findings and recommendations to Directors and Clinical Service Managers, and other appropriate hospice/home health personnel. 11. Assists in the screening and interviewing process of new employees and makes recommendations for employment of individuals. Assists in the orientation of new employees. 12. Assists personnel in the planning, implementation, and evaluation of in-service and continuing education programs. Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, goals, and objectives, both short and long range. 13. Complies with accepted professional standards and principles. 14. Assesses health needs of community and assists in the development of plans and programs to meet needs. Participates in public relation and community activities that promote the organization's role as an effective member of the hospice and home health care delivery system. Promotes customer service orientation to all hospice/home health personnel. 15. Participates actively in quality assessment performance improvement program. 16. Completes patient visits as needed to support field staff. 17. Completes and tracks on-site supervisory visits as required. 18. Performs record reviews and communicates findings and recommendations to appropriate organization personnel. 19. Review DME ordering and DME invoices for accuracy and appropriateness. 20. Review of pharmacy ordering for formulary options and appropriateness. 21. Review Medical Supply ordering. for formulary options and appropriateness. 22. Management of SHP outcomes and education of staff for improvement needs. 23. Management of HCAHPS outcomes and education for staff improvement needs. 24. Management of F2F process with NP and Medical Records. 25. Participates in administration on-call rotation. 26. Facilities IDT meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement. 27. Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director and hospice personnel. 28. Supports the scheduling team and ensures appropriate daily workload for field staff. 29. Helps support agency culture. 30. Creates best practice policies and collaborates with operations staff. 31. Support team by completing field visits, admission visits, and other needs when the team is in need of overflow support. 32. Participates in accountability meetings such as Clinical Accountability, Billing Accountability, Scheduling accountability. 33. Collaborates with the marketing team to provide in-services, best practice training for field staff when visiting facilities. 34. Support /back up other Clinical Leadership team members as needed. 35. Performs other duties and activities as delegated by the Clinical Director. Key areas of accountability and metrics include: Average Daily Census FT Weekly average Visits points Hospice Direct Cost Pharmacy Cost DME Cost Medical Supply Cost DSO Field Clinician turnover HIS Composite Score HCAHPS overall Composite Score State Survey results Internal Compliance Audit score Employee Engagement Survey results Employee Emergency fund These metrics, while varied in nature, collectively contribute to the overarching goals of providing high-quality patient care, maintaining operational efficiency, ensuring regulatory compliance, and fostering a positive and engaged workforce. JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities) Graduate of National League of Nursing accredited school of nursing with a BSN degree is preferred. Previous experience in hospice/home care setting and with two years management or supervisory experience. Proven ability to work within an interdisciplinary setting. Registered nurse with current licensure to practice professional nursing in the state. Complies with accepted professional standards and practice. Has excellent observation, good nursing judgment and communication skills. Understands hospice philosophy and issues of death/dying. Understands principles of pain/symptom management. The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job. Must have and maintain a valid driver’s license, maintain automobile insurance coverage and have access to an automobile. Complies and maintains current CPR certification.
