Position Purpose:

The Data Abstractor is a high-performing individual who is responsible for detailed medical record review and abstraction of clinical data required for quality measures reporting and/or departmental registries.

Position Duties (not all inclusive):

• Analyzes patient files, abstracts/collects relevant data from patient records and other sources utilizing the most current Core Measure Specification Manual.
• Accurately enters abstracted data into the appropriate database in a timely manner.
• Involved in the monthly nursing orientation process by introducing them to the required Core Measure elements.
• Responds timely to the Supervisor's request for information and assistance in a timely manner.
• Promote a culture of safety, Follow safety guidelines, Report visitor/resident/employee incidents immediately no later than end of business day.
• Abide by the Departmental and Facility Policies State Laws, Federal Laws, and Regulatory Agencies (i.e. OSHA, TJC, CMS, HIPAA).
• Ensures data is abstracted within established timeframes to ensure continual flow of information and increase efficiency of office visits facilitating patient entry into specialty services.
