The Human Services Coordinator III will oversee the Medical Case Management team under the Ryan White Program. This position will also function as a specialized case manager providing case management services to patients re-engaging in care. Reports directly to the Ryan White Nurse Manager.

Job Description

Job Duties:

35% - Function as the Specialized Medical Case Manager (SMCM) for patients re-engaging in care. SMCM works with patients to identify and utilize the patient’s current personal strengths to address and overcome barriers preventing the patient from continuously engaging in care and treatment. By allowing patients to prioritize their own service needs, the SMCM empowers patients to take charge of their care and build a plan that feels best suited to his/her individual needs. SMCM will maintain a caseload of no more than 20 patients at a time. The essential functions include, but are not limited to, the following:

  • Assists the Outreach Specialist with contacts to clients via telephone and conducting research to find updated contact information;

  • Works closely with Outreach Specialists as clients are located and prepared to re-engage agency services and medical care;

  • Interprets patient’s responses on barriers to care and treatment using the statewide tool Return to Care Assessment (RTC).

  • Customizes services through the development of an Enhanced Care Plan (ECP) with the patient to re-align the client’s perception/prior experience of service usefulness and accessibility barriers, allowing the client to set service priority

  • Communicate with management on necessary service enhancements

  • Emphasizes family/partner/other support systems

  • Provides goal-oriented, time-specific Specialized Medical Case Management activities and support to encourage the patient’s participation in his/her own care plan.

  • Works proactively with clients until they are ready for less intensive, traditional case management (i.e., SMCM must often take the initiative and follow the client’s medical appointment and other service referrals very carefully)

  • Identifies emerging barriers that may prevent each client from maintaining participation in care and treatment

  • Coordinates and links clients to services required to implement the ECP

  • Attend bi-monthly DHEC Outreach Committee Meetings, quarterly DHEC Outreach Program calls, and DHEC Outreach Program in-services when offered.

35% - The Medical Case Manager Supervisor will oversee the Medical Case Management team, supervising three to four case managers. Tasks include, but are not limited to:

  • Review and sign all required documentation for enrolling and maintaining patients in Medical Case Management.

  • Complete quarterly chart reviews to ensure compliance.

  • Develop and implement a process for assigning patients to a medical case manager.

  • Monitor and ensure that medical case managers are meeting productivity goals (monthly) and implement productivity improvement plans for those who do not.

  • Assure that medical case managers follow SC Ryan White Service standards Attend DHEC monthly Medical Case Manager meeting.

20% - Oversee continuing education and training for the Medical Case Management Team, including:

  • Maintain documentation of each medical case manager's completion of required training and continuing education;

  • Ensure adequate training of newly hired medical case managers;

  • Ensure that required DHEC training is completed and the final exam is passed within 18 months of hire;

  • Prepare medical case management for yearly DHEC audit.

10% - Effectively communicates with team members and physicians regarding patient recommendations based on professional assessment of patient abilities and resources. Effectively and proficiently uses Provide Enterprises (PE) and Epic along with Microsoft Outlook.

Preferred Experience & Additional Skills:

  • Knowledge of policies and procedures relating to establishing and maintaining community social service programs.

  • Knowledge of specialized needs in education and treatment for emotionally and/or environmentally handicapped clients.

  • Ability to establish and maintain effective working relationships with necessary agencies, organizations and interested public or private groups.

  • Ability to present clear and comprehensive oral and written reports.

  • Ability to identify and effectively utilize resources available in the community to accomplish objectives.

  • Ability to organize and maintain a caseload as necessary.

  • Ability to communicate effectively.

Additional Job Description

Minimum Requirements: A master's degree and three years professional experience in human services or social services programs.

(Continuous) Ability to match or discriminate between colors. (Continuous) Ability to determine distance/relationship between objects; depth perception. (Continuous) Good peripheral vision capabilities. (Continuous) Ability to maintain hearing acuity, with correction. (Continuous) Ability to hear and understand whispered conversations at a distance of 3 feet. Ability to perform gross motor functions with frequent fine motor movements. (Continuous) Ability to work in dusty areas. (Infrequent) Ability to be qualified physically (by medical personnel) for respirator use, initially and annually.
