DUTIES:

· Under the supervision of the agency System of Care Supervisor, provides wrap-around services to a caseload of 15 families. Collaborate with geo-assigned CFS workers to develop wrap-around teams for each family served by this project and determine family strengths and challenges. Develops a wrap-around case plan for each family in conjunction with CFS, as needed, and monitors progress. Maintain confidentiality with program participants. Attends all team decision meetings with the number one priority given to emergency staffing.

· Arrange and/or refer families to programs, collaborative service providers and other community agencies for needed support services to the families. Arrange support groups for families that include education and family oriented outing/activities. Conducts home visits as needed to provide follow-up services to families referred from CFS and to implement the wraparound process. Accrue data on success and recidivism and report to family, appropriate individuals and agencies with the objective of resolving the issues and closing the case. Document all contacts and expenditures related to each family.

Job Type: Full-time

Pay: $18.00 - $23.00 per hour
