Multnomah ESD is Hiring a Complex Needs Nurse Near Portland, OR

Over 135 MESD employees work in the Department of School Health Services. We are committed to ensuring the safety of students by providing the highest standard of nursing care in the school and community setting so that every student has the opportunity to achieve their educational potential. We are nurses, care coordinators, health educators, health assistants, administrative staff, outreach workers, screening and prevention specialists who accomplish this by providing and coordinating specialized health services that support the educational experience for students throughout Multnomah and Clackamas County. We support the Whole School, Whole Community, Whole Child (WSCC) model that combines and builds on elements of the traditional coordinated school health approach by responding to the call for greater alignment, integration, and collaboration between education and health to improve each child's cognitive, physical, social, and emotional development.

Classification/Range: Licensed

Schedule: 200 days per year

POSITION PURPOSE: 

To provide nursing services that facilitate the placement/school experience for students with complex health needs in all local district schools.

NATURE & SCOPE: 

This position is subject to board policies, administrative rules and procedures, department regulations, and applicable state and federal statutes. This position carries no administrative authority or responsibility.

ESSENTIAL JOB FUNCTIONS:

  1. To assess the health needs of referred students by:
  • Determining the extent of the health assessment required through record review and parent consultation.
  • Obtaining a health history from medical records and student/parent interviews.
  • Evaluating the student’s current health status through physical assessment and
  • student/parent interview.
  • Obtaining current medical information and orders from the student’s health care provider.
  1. To facilitate health services for identified students by:
  • Developing a Student Health Management Plan (SHMP) and providing a summary of health information including recommendations for the school program which includes a nursing care plan, emergency plans, delegation or assignment to school staff, transportation plans, IEP goals, and which may include teaching classroom staff to observe and report signs of identified concern to the R. N.
  • Re-evaluating student health status, identified health concerns, goals, and placement to determine if progress has been made, and health information is current, consulting with health care providers/parents to modify medical treatment and nursing management as needed.
  1. Coordinate the health care of students with special health needs with School Health Services staff RN by consultation and/or assignment including follow-up assignments responsibilities of SHMP.
  2. To act as a liaison between parents, teachers, community agencies, and medical providers, as needed, to ensure effective communication and make recommendations regarding the nature, scope, and intensity of services that will provide a safe school environment in the least restrictive environment for students in assigned districts.
  3. To participate as a member of the multidisciplinary team and make appropriate recommendations regarding student health/educational plans for IEP/IFSP.
  4. To participate in the evaluation of one's own job performance.
  5. To demonstrate commitment to maintain competencies for professional practice.
  6. To contribute to a positive and productive work environment.
  7. To participate in designated meetings as directed by the program Director/Supervisor.
  8. To consult with supervisor for assistance and/or direction as needed.

MINIMUM QUALIFICATIONS, SKILLS, ABILITIES: 

  • Willingness to work a flexible schedule.
  • Willingness to become knowledgeable in word processing.
  • Demonstrated effective communication, leadership, and time management skills.
  • Valid driver’s license and vehicle available for use.

DESIRED QUALIFICATIONS, SKILLS, ABILITIES: 

  • Knowledge of Multnomah County community resources and experience in a school health services program.
  • Fluent in Spanish, Vietnamese, Russian, Hmong, Chinese or other non-English languages

EDUCATION & EXPERIENCE:

  • Valid Oregon license to practice as a Registered Nurse and
    • minimum of two years of experience within the last four years working in a school/clinical setting with children who have chronic or technology-supported health problems or
    • A combination of education and experience that would provide the applicant with the desired skills, knowledge, and ability required to perform the job
