JOB DUTIES
Maintain the integrity of the electronic medical record, financial record, and various other downstream applications through analysis and audit of the clinical and financial information to ensure every patient has one unique identifier to ensure patient safety and compliance are not comprised.
Perform analytical reviews of clinical, financial, and demographic data utilizing all applicable computer systems to inform decisions and next steps for patient and/or encounter combines. Identify discrepancies in the registration or billing data to prevent patient harm, billing errors, or compliance issues.
Perform medical record number merges, patient registration corrections, and medical record overlay corrections in accordance with organizational policies and procedures.

Work identified duplicate/overlay cases using designated reports, work queues, e-mail communications, phone calls, and interdepartmental mail. Collaborate with various levels individuals throughout the organization, including clinical staff, leadership, administrative staff, and others to coordinate merge functions and data corrections.

Proactively and independently solves problems and operates with a high level of accuracy. Exhibit excellence in communication and follow through with data, customers, and peers, as there is a high level of patient safety and compliance risk associated with review and remediation of duplicate patients and encounters.

Ensure all parties involved in making corrections in any disparate systems are promptly notified with follow through to ensure all corrections are executed in a timely manner. Communicates merges and system issues with Data Integrity team and HIM Leadership.

Use knowledge of the financial system, both hospital and professional areas, in order to move insurance, appointments, and visits to the appropriate patient/MRN as part of the combined workflow.

Notify the appropriate individuals to update upstream and downstream applications to ensure that data flow is not interrupted by a combine.

Follow compliance regulations and report the impact of duplicate records as required for potential breach investigation.

May complete unit/department specific duties and expectations as outlined in department documents.

Knowledge, Skills, & Abilities
Experience working with various healthcare information systems, with an emphasis on resolving duplicate medical record numbers.

Ability to analyze complex information and use problem solving skills and analytic skills to determine appropriate solutions and necessary merges to protect the integrity of the EMPI.

Excellent verbal and written communication skills.

Strong customer service skills.

Familiarity with EMR and how information flows to and from it.

Knowledge of patient registration, visit creation, patient scheduling, and loading insurance.

Qualifications

Associate's degree in health information technology/management or other health related field. One (1) year of experience as a data analyst or performing EMPI remediation tasks for a healthcare organization may be substituted for the education.

Three (3) years of experience in health information management or a related health care field.

Preferred Qualifications
Additional license/certification requirements as determined by the hiring department.
