(MPAC@HOME) Monumental Post Acute Health At Home is Hiring a PRN Home Health Nurse Near Delaware, PA

Job Overview:
We are seeking a dedicated and compassionate Registered Nurse to join our healthcare team. Under general supervision of clinical management, provides skilled nursing care to assigned patients and directs Licensed Practical Nurses and Home Health Aides to provide quality care. Assesses, plans, implements, evaluates total patient care, and coordinates patient/family teaching. Assures compliance with nursing standards, federal, state, and local government regulations, and company clinical policies. Provides goal directed care leading to specific outcomes. May be on call for after-hours work.

Responsibilities:
1. Provides professional nursing care to assigned patients.

2. Assesses patient’s physical and emotional status, level of competency, home environment, safety factors, and family support available to assist with care and needs of patient.

3. Providing initial and ongoing assessment to determine level of functioning, including OASIS Assessment at appropriate time points.

4. Provides information on patient rights, reimbursement procedures, and patient/family rights.

5. Assesses and evaluates patient status, reviews, and implements plan of care, reevaluates needs, and revises plan of care as necessary.

6. Observes patient for changes in condition and communicates/coordinates care with attending physician or allowed practitioner and clinical management.

7. Developing and revising the plan of care in consultation with the physician or allowed practitioner and other care-team members.

8. Responsible for planning, evaluation, and treatment implementation of assigned caseload.

9. Preparing clinical progress notes.

10. Provide supervision, ensuring Licensed Practical Nurses and Home Health Aides provide quality care.

11. Initiates and applies appropriate preventative, therapeutic and rehabilitative nursing procedures and techniques as ordered by the physician.

12. Administers medications and intravenous solutions.

13. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and independence.

14. Maintains patient clinical records and reports as required.

15. Supervision, ensuring Licensed Practical Nurses and Home Health Aides provide quality care.

16. Evaluating outcomes.

17. Planning Discharge.

18. Participates in on-call rotation as necessary.

Job Conditions:

1. Position is stressful in terms of meeting deadlines.

2. Position is primarily a field role which essentially involves driving, standing, stooping, and walking, as well as an inordinate amount of documentation and telephone communication.

3. The work requires moderately heavy physical exertion on a regular and recurring basis such as: extensive driving, assisting patient in transfer activities (wheelchair, to bed, to tub, to commode) and providing substantial support to individuals in ambulation therapy.

4. The ability to utilize a PC, calculator, telephone, and other related office equipment.

5. Travel is required, by car or airplane to patient homes, facilities, local, or state seminars, conferences, or meetings.

6. The ability to communicate well, both verbally and in writing is required.

Requirements:
1. Licensed Registered Nurse in state of Pennsylvania.

2. Graduate of an accredited school of nursing. BSN degree preferred.

3. At least one year of professional nursing experience in patient care required.

4. Ability to work independently and manage time effectively.

5. Strong interpersonal, organizational, and problem-solving skills.

6. Solid computer skills; prior experience with electronic medical records (EMR) preferred.
