Job Title: Community Health Worker
Reports to: Site Manager
Salary: Starting at $16.40/hr (salary commensurate with experience and relevant certifications)
Job Summary:
· Responsible for helping patients navigate and access community services, and work closely with medical and mental health providers and case managers to ensure patients have comprehensive, coordinated, and integrated care.
· Obtain and follow-up on patient data for value-based care processes.
Job Qualifications:
· Licenses/Certifications/Requirements:
o High School Diploma or GED
o CHW or Care management certification preferred
o Previous CHW, Care Coordinator or MA experience preferred
o Current Basic Life Support (CPR) certification required within 6 months of hire
o Proof of current immunizations will be requested upon hire
· Experience in hospital or clinical setting preferred.
· Bilingual (Spanish/English) preferred.
· Excellent interpersonal communication and customer service skills.
· Ability to identify, prioritize and complete tasks with limited supervision.
· Reliable transportation. Limited travel to other sites for cross-coverage is possible (<15 miles).
· Basic computer skills, including Excel. Experience with EMR documentation and medical terminology a plus.
Essential Duties:
· Assist Wasatch Behavioral Health and MCHC patients to access community and medical services.
· Act as liaison between patients and other medical offices to ensure coordination of care.
· Participate in the integrated care model to assist patients obtain both medical and mental health services.
· Work closely with medical providers, mental health providers, and case managers.
· Assist the MCHC Health Connections Coordinator with Value-based care initiatives including Transitional Care Management, Annual Wellness visits, and hospitalization/ER follow-up calls.
· Attend integrated care and value-based care coordination meetings as directed.
· Provide clear and timely instructions to patients as directed by provider.
· Daily follow-up on messages.
· Document all care and communication in the EMR.
· Maintain skills and knowledge through training, required meetings, and supervisor check-ins.
· Follow MCHC policies, procedures, and standards. Display a professional appearance and demeanor.
