Mountain View Healthcare Center, a leading provider of Skilled Nursing Services located in Santa Clara County, CA, is presently seeking an experienced full-time Social Service Director to join our team.

Hourly Rate: $24- $30 (based on experience)

DUTIES:
Provides leadership, direction and supervision to the Social Work staff, ensuring continuity and consistency of participant care planning across all centers.

Sets standard criteria and process, and performs utilization review of participant care plans.

Ensures delivery of quality services by overseeing the department’s compliance with regulatory requirements, adherence to policies and procedures, and leading continuous process improvement and risk management efforts.

Provides individual case management to participants as needed.

Organize and facilitate department meetings.

Establishes, develops, and maintains working relations with community agencies and organizations.

Other related duties as assigned.
