Job Description
Provides primary counseling for caseload: this includes individual, group and family intervention as needed
Responsibilities
1. Develops and implements individual treatment plans for assigned patients
2. Implements or modifies discharge planning
3. Communicates with referral sources, insurance companies and managed care companies
4. Documents patient progress verbally with staff and written in medical chart or as needed
5. Participates in staff meetings, supervision and rounds
6. Prepares and delivers lectures on chemical dependency
7. Participates in crisis intervention
8. Facilitates psychiatric evaluations and/or referrals
9. Participates in any activity as directed by the Evaluation Supervisor
10. Conducts assessment/evaluations as directed
Qualifications

• High School diploma or GED
• Minimum two years counseling experience in long term C.D. drug free program.
