Job Overview:
We are seeking a compassionate and dedicated Registered Nurse to join our Home Health team and work with us per diem. As a Registered Nurse - Home Health, you will provide skilled nursing care to patients in their homes. The nurse will primarily assist with conducting nursing assessments and supervisory visits in the patient’s home. Scope of responsibility also includes collaboration with staff and effective management of resources to deliver successful patient outcomes. Our service areas for nurses are in the following counties throughout New York State: Queens, Kings, Bronx, Nassau, Staten Island and New York. Responsibilities:
- Conduct initial assessments and develop individualized care plans for patients
- Administer medications and treatments as prescribed by the physician
- Monitor and evaluate patient progress, making necessary adjustments to the care plan
- Provide education and support to patients and their families regarding their condition and self-care techniques
- Collaborate with other healthcare professionals to ensure comprehensive and coordinated care
- Document patient care activities accurately and timely

- Collaborates with other clinical disciplines, involves patients, family, and care partners in the development of the plan of care
- Adhere to all applicable laws, regulations, and policies

Experience:
- Minimum of 1 year of nursing experience, preferably in home health or a related field
- Experience working with geriatric patients is highly desired
- Familiarity with ambulatory surgery center protocols and procedures
- Proficient in diagnostic evaluation techniques
- Knowledge of aseptic technique for wound care and catheterization
- Basic life support (BLS) certification preferred
- Experience with electronic health record systems, such as HHAexchange, is preferred
- Experience in behavioral health or acute pain management is a plus

- NYS licensed and currently registered as a Registered Professional Nurse

- Excellent organizational writing, communication and interpersonal skills.

- Successful track record of working independently while taking on increased or various responsibilities.

- Demonstrated ability for Assessment, Evaluation, and Interpretation of Medical Information and Health Care Planning

Pay: $53.00 - $57.00 per hour
