Requirements
- Doctor of Dental Medicine (DMD), Doctor of Dental Surgery (DDS),
- Preference given to MD candidates
- Certificate in Oral Maxillofacial Surgery
- Board Certified or Board Eligible
- License/credentials applicable to the state of California
- CPR/BLS Certification
- Registration with DEA for controlled substances
