Position Overview: We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our home care team. The LPN Home Care Nurse will be responsible for providing skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN) or physician. The ideal candidate will have a strong clinical background, excellent communication skills, and a genuine desire to make a positive impact on the lives of our patients.

Responsibilities:

• Conduct initial patient assessments and develop individualized care plans in collaboration with the RN and other healthcare team members.
• Provide skilled nursing care, including wound care, medication administration, and monitoring of vital signs.
• Educate patients and their families on disease management, treatment plans, and medication regimens.
• Coordinate care with other healthcare providers, including physicians, therapists, and social workers, to ensure continuity of care.
• Document all patient care activities accurately and in a timely manner.
• Evaluate patient responses to treatments and interventions and modify care plans as needed.
• Provide emotional support and encouragement to patients and their families.
• Maintain a safe and clean environment for patients and adhere to infection control protocols.
• Participate in ongoing training and professional development activities to stay current on best practices and regulatory requirements.

Qualifications:

• Current LPN license in Maryland
