RN – Weekends, 8:30am-4:30pm

We are currently seeking a Weekend RN's to care for home care patients in our service area. The RN provides clinical services to patients with functional limitations that affect their ability to carry out activities of daily living. The RN also provides direct care, support, and education in the home under the direction of the attending physician and in collaboration with the family, LPN, therapy, home health aide, and any other disciplines. Assignments require considerable independence of professional judgment and are carried out in accordance with best practice standards and established policies, procedures, rules and regulations of the agency.

Essential Duties and Responsibilities:

• Assess referred patients for clinical services.
• Develop and educate patients and their care givers on the plan of care.
• Thoroughly complete all required documentation.
• Possess excellent interpersonal, communication and time management skills.
• Willingness to travel within our service area.

Education and Experience:

• Licensed as a RN by the State of New Jersey.
• Certification for CPR.
• At least 2 years of acute care experience.
• Experience with Homecare Homebase a plus but not required.
