PHYSICAL LOCATION:

Moody Neurorehabilitation Institute Clear Lake Campus

GENERAL STATEMENT OF DUTIES:

The schedule for this position is four - 10 hour shifts, Sunday - Wednesday. This is a set schedule. The Licensed Vocational Nurse (LVN) is a patient care provider. The LVN performs a variety of nursing functions and closely assesses patients for changes in medical or nursing status under the direction and supervision of the Registered Nurse and Medical Director. The LVN does provide care to adults with varying degree of acquired brain injury (ABI,) who may or may not have sustained other injuries. The LVN will be responsible for supervision and documentation of the patients' medical/nursing needs until discharge. Among these duties are observing and documenting patients' behavior, modeling appropriate behavior for the patient, interacting with the patient, providing training to staff in caring for the patient, ensuring patient safety, and reinforcing treatment plans and Moody Neuro policies. The LVN communicates to Registered Nurse, Medical Director, Program Director, and families regarding the patients' medical care. The LVN will provide direct billable education to patients regarding their specific medical needs: i.e., diabetes, medical hygiene, medication management, healthy living, and healthy food choices, etc. The LVN will also provide health related educational support for the clinical and therapeutic assistants as required by CARF, CRS and HHSC. The LVN will also provide clinical and therapeutic assistant competencies related to health care. The LVN will uphold the mission, philosophy, and goal of the organization while maintaining strict confidentiality of personnel and client data and information in the performance of assigned duties.

MAJOR DUTIES AND RESPONSIBILITIES:

  • Specified nursing services as needed by Moody Neuro patients.

  • Assist in emergency situations i.e., fire alarm and drill procedures, and provides first aid treatment in the case of illness or injury, while adhering to triage procedures.

  • Educate all staff on the policies and procedures related to all health and safety issues and provide/maintain all documentation of related education.

  • Read, understand, and ensure compliance with CARF, HHSC, CRS, and Moody Neuro regulations related to health and safety.

  • Monitor patient self-administration of medications and document ability to perform this function.

  • Provide documentation of observation of patient responses as needed.

  • Provide patient education in the individual or group setting weekly.

  • Serve as a role model at all times for the patients and residential staff.

  • Attend in-services, workshops, staff meetings, and seminars, as directed.

  • Maintain confidentiality of both patient and staff issues.

  • Provide a safe and secure environment for patients and staff.

  • Admission/Discharge duties in accordance with Moody Neuro organizational policies, CARF, and other accrediting/licensing bodies.

  • Assist in the training of residential staff as directed by the Medical Director or Program Director.

  • Assure medical documentation is in accordance with licensing agencies and that all co-signatures required by RN are completed.

  • Schedules and administers initial TB Test with new employees and patients. Documents results and administers annually.

  • Conducts New Hire drug screenings or incident follow up drug screenings as instructed by the Program Director.

  • Scheduling outside medical appointments assigned by the Medical Director while patients are admitted to Moody Neuro.

  • Other duties as assigned.

Organizational Duties:

  • Maintain medical documentation, including, but not limited to, medication monitoring sheets, incident reports, and medical consults.

  • Act as a liaison between the Registered Nurse, Medical Director, Program Director, and the staff related to health and nursing issues.

  • Maintain nursing, health, and safety procedures according to Moody Neuro policies.

  • Maintain appropriate on call records for timecard.

Required Knowledge, Skills and Abilities

  • Current Texas state license as an LVN.

  • Knowledge of TBI and related syndromes.

  • Ability to work well with other people in a clinical setting.

  • Ability to stimulate and motivate persons with problem in motivation.

  • Knowledge of social customs and responsibilities.

  • Ability to demonstrate flexibility, innovation, and good judgment.

  • Ability to receive and follow instructions accurately.

  • Knowledge of philosophical and ethical considerations in rehabilitation practice.

  • Ability to pass CPR certification requirements.

  • Ability to communicate accurate and pertinent information through documentation.

  • Ability to execute emergency procedures with regard to fire drills, emergency evacuation, and triage.

  • Ability to work rotating shifts and hours.

  • Ability to assist clients with transfer into/from a wheelchair.

  • Ability to obtain NCI certification.

Education:

  • Graduate of an accredited school of nursing.

  • Hold a current license to practice as a Licensed Vocational Nurse in the state of Texas.

  • Required CPR certification.

  • Required Texas drivers license.

Experience:

  • Prefer 2 years post-graduate experience in nursing, preferably in the fields of rehabilitation, patients with cognitive deficits, or mental health issues.

    • Depth perception - three-dimensional vision. The ability to judge distance and space relationships so as to see objects where and as they actually are they actually are.
    • Field of vision - the area that can be seen up and down or to the right or left while the eyes are fixed on a given point.
    • Accommodation - adjustment of the lens of the eye to bring an object into sharp focus. This item is especially important when doing near-point work at varying distances from the eye.
    • Color vision - the ability to identify and distinguish colors.
