Position Summary:

Provides full range of social services to patients and families, including counseling in issues related to death and dying, coping, crisis intervention and community and economic resources. Serves as a primary member of an interdisciplinary team, assisting other disciplines on the team in understanding the significant social and emotional factors related to the patient’s and family’s comfort. Explains hospice services and benefit to referred patients and families; obtains consents for hospice care. Works in collaboration with the other primary team members to coordinate care for the patient and family.

Duties and Responsibilities:

• Evaluates the social, emotional and environmental factors to determine the capacity of the patient and caregivers to cope with terminal illness and death.
• Participates in the completion of a psychosocial assessment delineating specific problems, goals, and interventions.
• Coordinates with interdisciplinary team members to develop and implement a plan of care that includes potential problems and areas in social work interventions.
• Provides initial assessment of hospice patients.
• Presents hospice services to potential hospice patients and to other healthcare professionals.

Minimum Qualifications:

• Masters of Social Work from an accredited graduate program.
• Licensed Social Worker with the state of Maryland. LCSW-C preferred.
• Minimum of two years post-graduate clinical experience.
• Ability to work with patients and families dealing with life-threatening illness required.
• Excellent listening and communication skills and effective interpersonal skills.
• Previous hospice experience preferred.

Pay: $30.00 - $40.00 per hour
