Stillwater Center is a home for children and adults who are residents of Montgomery County, have the most severe and profound intellectual disabilities, are physically challenged, and/or have significant medical needs.

A Habilitation Provider at Stillwater Center is required to have working knowledge of resident care; under general supervision, provides basic care of residents with IDD/DD and trains residents to function independently in self-help living skills based on individual habilitation plans; participates in the development and modification of plans.

Summary of Job Duties

Provides direct individual habilitation to residents on a daily basis, following the Individual Habilitation Plan in the Programming areas: 1) Self-Help (i.e., toileting, bathing, teeth brushing, face/hand washing, grooming, dressing and dining behavior); 2) Communication (i.e., verbal, vocal, gestural, manual and augmentative expressive skills and understanding directional requests for receptive skills); 3) Socialization (i.e., appropriate interaction with others, participatory skills, developing friendships and community behaviors); 4) Leisure/Recreation (i.e., independence with age-appropriate instructional/leisure materials, developing interest in sensorimotor activities, participating in community events and cooperative play or recreation activities. These areas of programming are nonexclusive as based on individual needs/wants per the Individual Habilitation Plan. Responsible for program accountability with daily data progress recording; interacts in a positive manner with residents at all times.

Ensures the safety and wellbeing of residents: supervises and records food and liquid intake, records elimination, provides positioning to prevent deterioration every two hours or more as needed, monitors vital signs, observes, notes and reports any significant changes in residents behavior or condition for medical attention; performs basic nursing functions such as taking temperatures, other approved procedures; records medical data for required documentation; ensures physical safety of residents by direct supervision of caseload or area of programming at all times; monitors residents safety in room or bed with bed rails up when not directly involved in programming as well as when programming or during unstructured times in bathrooms, dining rooms, halls, outside and inside environments.

Assists in the development of the Individual Habilitation Plan; attends Individual Habilitation Plan meetings, staff in-services and staff meetings; escorts residents to scheduled activities and appointments; maintains clothing and personal items of residents; assists in cleaning areas of programming used, wheelchairs and other adaptive equipment of residents; may be required to transport and/or escort residents to and from community activities and scheduled appointments.

Maintains a clean and orderly home; assists with serving meals. Makes administration aware of equipment needs and repairs. Makes minor household repairs; Keeps records of residents’ progress; maintains accurate and up-to-date records, e.g., medication sheets, menu planning, daily log reports, etc. Operates motor vehicle as assigned in a safe manner, monitoring maintenance of vehicle and cleanliness. Maintain and disperse medications responsibly, recording administration of medication. Carries out daily living and extracurricular activities and transportation, e.g., grocery shopping, doctor appointments, etc. Attends and participates in scheduled training sessions, in services and staff meetings. Establishes appropriate relationships with the families and guardians of the residents. Becomes familiar with emergency procedures.

Minimum Qualifications and Requirements

Highschool diploma or GED, six (6) months experience in habilitation field working with handicapped, severe to profound developmentally disabled or six (6) months basic resident care nurse aide experience in a long-term care facility or six (6) months home health experience with CNA or STNA certification or equivalent combination of education and/or experience in disabled service. Must have valid Driver’s License with acceptable driving record and show proof of insurance.

1st Shift (6:00 am - 2:00 pm) or 2nd Shift (2:00 pm to 10:00 pm) or 3rd Shift (10:00 pm to 6:00 am)

New Employee Orientation training in the care of residents with developmental disabilities and CPR and First Aid Certification must be successfully completed during initial probationary period; must be certified in medication administration.

  • All full-time, part-time, temporary, and intermittent employees of Montgomery County working at Stillwater Center are required to be fully vaccinated against COVID-19. New hires must provide a copy of their vaccination card to the Stillwater Center HR Manager prior to their employment start date or receive an approved exemption from the Director of Human Resources.
