Position: Palliative Care Physician

Monogram Health is seeking a full-time physician, specialized in palliative care, to join the team due to continued nationwide growth. This physician will be an in-market specialist and connect with CKD and ESRD appropriate for conservative care and symptom management and engagement with patients who have multiple complex co-morbidities. This is an opportunity to practice medicine in a forward-looking, evidence-based environment with ample growth opportunity, alongside nephrologists committed to raising renal care outcomes nationwide. The Palliative Care physician will be integrated into the pod with the pod Medical Director and meets with patients in their home as well as through telemedicine.

Roles & Responsibilities

  • Collaborate with Pod Medical Director to proactively identify CKD and ESKD members in predefined geographic region to build a patient panel 
  • Own the outcomes for patient panel, driving both predetermined clinical and financial KPIs through interventions outlined below
  • Treat panel of patients both in-home and virtually, as necessary, to drive healthcare outcomes
  • Reduce or control pain or debilitating symptoms on patients who have selected a palliative care pathway 
  • Round on patient panel (if admitted) in selected hospitals within a defined geographic market
  • Refer members to appropriate specialists to drive clinical care (Monogram-based specialists or locally-aligned specialists)
  • Conduct meaningful end-of-life and treatment options discussions with patients
  • Develop key relationships with local providers (PCP, nephrologists, specialists) to improve market alignment
  • Participate in monthly sessions with RMD/CMO in areas of KPI and expectations around KPIs
  • Review Annual Wellness Visit assessments to ensure proper coding and provision of medical diagnoses across multiple comorbid conditions
  • Assist in the training of NPs and other providers to provide high-quality care

Position Requirements 

  • 1 year treating patients with CKD and ESRD 
  • Desire to practice in a forward-looking renal care environment focused on population health management 
  • Demonstrated experience applying evidence-based clinical criteria 
  • Experience in geriatrics and/or palliative care 
  • Hospital privileges at one or more local acute-care facilities
  • Comfort and skill in end-of-life discussions and advance care planning 
  • Experience with high-need Medicare Advantage and managed Medicaid populations, as well as commercial populations 
  • Experience integrating acute and behavioral health care and appreciation of social determinants of health and the value of social workers 
  • Expertise in managing medication panels for complex patients 
  • Current state medical license without restrictions to practice and free of sanctions from Medicaid or Medicare. Willingness to become licensed in multiple states as company and patient needs require. 
  • Doctorate Degree in Medicine 
  • Board Certified in Hospice and Palliative Medicine
