LPN - Mohawk Valley Health System Rehabilitation and Nursing Center (Multiple shifts available)

Responsibilities

• Gathers data and implements the nursing care of residents within the skilled nursing setting with the supervision of a Registered Nurse.
• Responsible for ensuring continuity of care of the residents between shifts by providing direct care.
• Supervising the care given by CNAs and supportive staff members.
• Participates in resident and family teaching.
• Maintains role as resident advocate with a focus on the facility’s mission.

Skills

• Proven experience as LPN or similar role
• Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
• Familiarity with nursing home health, safety and sanitation standards and procedures
• Understanding of confidentiality obligations and nursing best practices
• Practical Nurse licensure in NYS
• One to two years experience in geriatric or acute care setting preferred.
• BLS certified.

Job Types: Full-time, Part-time

Pay: From $19.96 per hour
