JOB DESCRIPTION

DIRECT SUPPORT PROFESSIONAL

REPORTS TO: DIRECT SUPPORT SUPERVISOR/PROGRAM ANALYST

DEPARTMENT: ODP/IDS

POSITION SUMMARY:

The Direct Support Professional is responsible for a variety of duties related to the overall care of individuals with developmental disabilities. This includes direct care of program individuals and implementation of Individual Support Plans (ISP). Direct Support Professionals are required to perform all duties in accordance with Pennsylvania Office of Developmental Programs regulations and MOG policies and procedures.

POSITION QUALIFICATIONS:

High school diploma/GED required

  • 2 years’ experience providing direct care to patients with intellectual and developmental disabilities
  • Valid drivers license
  • Current First Aid and CPR certification (preferred not required)
  • Excellent organizational, interpersonal and communication skills
  • Compassionate about the care and well-being of others
  1. Evidence of sympathetic attitude toward care of the sick
  2. Demonstrated ability to read, write, and carry out directions
  3. Evidence of maturity and ability to deal effectively with job demands
  4. Good verbal and written communications skills required
  5. Have (24) hours of in services per year
  6. Participates in professional meetings when directed
  7. Shall have a criminal history check conducted prior to being offered permanent employment with this agency
  8. Ability to work closely supervised to ensure competence in providing individual care

TRAININGS

  • Completes and maintains all required ODP mandated trainings;
  • Orientation and training to work with individuals goals and obtain the outcome results required
  • Attends meetings and training sessions as required by MOG;
  • Completes annual trainings (minimum of 24 hours) related to individual ISP renewal and change in goals

DAILY ACTIVITIES

  • Provides transportation to and from activities when required;
  • Assists in the implementation of individual plans for individuals;
  • Assists individuals in the pursuit of recreational, social and community activities based on their preferences and individual plans;
  • Assists and encourages individuals in the establishment of social networks;
  • Promotes participation and involvement in community outings;
  • Works all assigned schedules, including but not limited to: arriving and departing on time, requesting time off in advance, and notifying your supervisor of changes to your assigned work schedule;
  • Ensures each individuals’ rights are protected;
  • Treats individuals with dignity and respect;

ACTIVITIES OF DAILY LIVING

  • Reminds individuals with the use of medication
  • Observes individuals for any changes in behavior or health that may have resulted from medication administration;
  • Report any incident with the individual immediately as contained in MOG incident management policy
  • Ensures individuals are dressed neatly and appropriately and are groomed at all times;
  • Assists and encourages individuals to care for immediate personal needs such as toileting, washing their hands, and eating;
  • Prepares nutritional meals based on individual needs and preferences;
  • Encourage individuals to assist in the preparation of meals and snacks as appropriate to their individual plan and behavior plan;
  • Teaches individuals daily skills such as household chores, laundry, cleaning, and washing dishes, as appropriate according to their individual plan;

COMMUNICATION

  • Observes individuals for evidence of injury or bruises and evaluates for changes in emotional and physical status;
  • Reports any noted problems to the Direct Support Supervisor immediately
  • Reports all medical related incidents to the Direct Support Supervisor;
  • Acts as a representative for MOG Home Health Care in a professional manner when required, to families, service coordinators, State officials, community partners, local businesses, and employers;
  • Represents MOG Home Health Care in a professional manner to families, service coordinators, State officials, and community partners when required;
  • Maintains individual confidentiality;

TEAMWORK

  • Assists the Direct Support Supervisor in the preparation for the annual Individual Planning Meeting;
  • Treats all individuals and coworkers and with dignity, respect, and fairness;
  • Provides support to other team members when individuals are having behavioral issues;

DOCUMENTATION

  • Documents all activities in individual service notes and update the activity charts when required;
  • Obtain individuals health documents and provide copies to MOG (i.e. physical, discharge documents, doctors notes, appointments, specialist recommendations,);
  • Obtain health related education documents from Direct Support Supervisor to educate individuals and families
  • Attends health fairs and other trainings, meetings that are scheduled for the benefit of individuals
  • Ensures that all documentation is completed accurately and in a timely manner;
  • Clocks in and out before and after every shift and submits Leave Request forms when applicable

PHYSICAL REQUIREMENTS:

  • Visual/hearing ability sufficient to comprehend written/verbal communication.
  • Ability to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing.
  • Ability to lift/push/pull 50lbs.
  • Ability to work on holidays and during inclement weather
  • Is neat in appearance and practice, with good personal hygiene
