Job Title: Social Worker - Case Manager

Responsibilities:

• Perform high-level triage for patients, focusing on identifying complex psychosocial issues and community service needs within 24 hours of admission.
• Receive RN Care Manager referrals based on identified Social Work Triggers.
• Assess barriers early in patient stays and formulate comprehensive plans in collaboration with patients, families, and healthcare team members.
• Identify patient and family needs, educate them on community resources, and arrange necessary services to support successful transitions of care.
• Develop discharge plans in consultation with patients, families, physicians, and healthcare teams, ensuring safe and timely transitions to the next level of care.

Requirments:

• Active license of LMSW issued by MI State
• BLS Certification
• 3 years of Acute care experience or Social work experience

Schedule:

• Mon - Fri
• Working Days: 5
• Hours Per Day: 8
• Weekly Hours: 40

Job Type: Contract

Pay: $70.00 - $75.00 per hour

Expected hours: 40 per week

• Master's (Preferred)

Experience:

• Acute care: 3 years (Preferred)
• Social worker: 3 years (Preferred)

License/Certification:

• LMSW license issued by MI State (Required)
• BLS Certification (Required)

Ability to Relocate:

• Petoskey, MI 49770: Relocate before starting work (Required)
